Prior to enrollment, each child shall have on file:
- A legal birth certificate (not the form you received at the hospital)
- Social security card
- Physical on KY school forms (the doctor’s office will have these forms)
- Current immunization certificate
- Eye examination by an optometrist or ophthalmologist
- Proof of residency such as an electric bill with your name and address (this refers to in-district families)
- Guardianship or custody papers if applicable
Tuition families will also need to complete the tuition contract with a deposit.
